Form 3B Evaluation
⚠️ CRITICAL: Form 3A Structure Part 1: Attach most recent Form 1 Part 2: For EACH outcome, complete all 5 elements:
1️⃣ Learning outcome (copied from Form 1)
2️⃣ Methods used (from Form 2)
3️⃣ Performance targets (from Forms 1 & 2)
4️⃣ Results with analysis (from Form 2, expanded)
5️⃣ Program improvements planned (NEW—this is the key!)
Part 1: Attach Current Assessment Plan
✓ Attach your most recent Form 1
This provides context for reviewers to understand what you originally planned
Part 2: The 5-Element Structure (Repeat for Each Outcome)
1. Learning Outcome Assessed
Copy the exact CSLO from Form 1 verbatim
"Phy Sci 101 CSLO #6: Integrate the relationship between solar radiation and Earth in generating weather."
2. Methods Used
Pull from Form 2—what you actually did
"Brightspace Quiz with 3 criterion-referenced questions administered in Weeks 13-16. Q2 specifically assessed this CSLO, asking students to explain the relationship between solar angle and seasonal temperature."
3. Performance Targets
State the benchmark from Form 1 (should match Form 2)
"Target: 100% of students taking assessment would score 50% or higher. Minimum 80% participation expected."
4. Results with Analysis
Two components required:
Results: 108 of 119 students (91%) correctly answered Q2. 65% overall participation (119/183 enrolled).
Analysis: Students demonstrated strong understanding of basic seasonal temperature causes. The 91% success rate exceeded our 50% benchmark. However, participation fell short of 80% goal. Analysis of non-participants shows 34% withdrew before Week 13, suggesting retention issues earlier in semester may impact assessment completion rates.
5. Program Improvements Planned ⭐ MOST IMPORTANT
This is where Form 3A adds value. Make plans:
Specific: Clear, concrete actions
Connected: Address your findings directly
Actionable: You can actually implement them
Measurable: You'll know if they worked
❌ Weak Improvement Plan:
"I will teach this material better next time and make sure students understand it more."
Problems: Not specific, not measurable, no concrete actions
✅ Strong Improvement Plan:
"To improve hemisphere understanding (Q3 weakness): 1) Add PhET 'Seasons' simulation in Week 10, 2) Create guided worksheet comparing N/S hemispheres, 3) Add low-stakes formative quiz in Week 11 to check understanding before final assessment. Will measure impact by comparing Q3 performance in Spring 2025 vs Fall 2023."
Types of Improvements to Consider
Curricular/Instructional:
Revise content sequence or organization
Add or modify learning activities
Change instructional methods
Adjust pacing or timing
Integrate more active learning
Assessment-Related:
Revise assessment instruments for clarity
Modify rubrics to be more specific
Adjust timing of assessments
Add formative assessments before summative
Change assessment format or modality
Student Support:
Increase office hours during key periods
Create supplementary resources or tutorials
Establish peer tutoring or study groups
Provide more frequent feedback on progress
Add review sessions before major assessments
Program-Level:
Recommend prerequisite changes
Suggest course sequencing adjustments
Propose policy modifications
Address program-wide patterns

💡 Form 3A Success Formula: Data (Form 2) + Thoughtful Analysis = Concrete Action Plans (Form 3A) → Implementation → Evidence of Effectiveness (Form 3B). Don't just say what you'll do—explain HOW and WHY, and describe how you'll know if it worked.
⏰ Timeline Tip: Be realistic. Don't promise to implement 15 major changes. Focus on 2-3 high-impact improvements you can actually complete before your next assessment cycle.
